OP-C-4 HEALTH SERVICES
OBJECTIVE
OVERVIEW
A. DEFINITIONS
Fee-for-service rates include charges for office visits, nurse assisted clinics and pricing that is higher than fees charged to currently enrolled, fee paying, FSU students (for lab, x-- ray, pharmacy, injections and other medical supplies).
Health Fee card is a card that puts the fee for service patient on the same fee schedule as a fee-paying student. In other words, Health Fee card purchasers do not pay for office visits for injuries or illnesses. Cards are sold by the semester, are pro-rated over the course of the semester and also cover student counseling center visits.
Fee paying students are students who pay health fees as part of their tuition. Some students do not pay fees (example, state employees). Students who have their tuition waived are not eligible to use the health center services. Many graduate students are fee waived, but are eligible and do pay fees so they may use the health center. When they pay fees, it will register in the system and show them as "fee paid-eligible for services."
B. FSU STUDENTS
Currently enrolled, HEALTH FEE PAYING*** (fee paying) students are eligible to be seen for all medical services provided. There is NO CHARGE for office visits for injuries or illnesses. They will be charged for use of lab, x-ray, injections, pharmacy and other medical supplies used.
The Student Counseling Center is now located in the Student Life Building. Its services are available for currently enrolled fee-paying students.
C. FSU SPOUSES AND DEPENDENTS 13 YEARS AND OLDER
Spouses and dependents 13 years of age and older, of currently enrolled, fee paying FSU students*** are eligible to be seen. They ARE CHARGED for office visits. The FSU student sponsor must accompany the spouse or dependent unless prior arrangements are made with the business office. They will be charged fee-for-service rates*. They are eligible to purchase a HEALTH FEE CARD** each semester that places them in the same paying status as fee-paying, currently enrolled FSU students.
Students who were enrolled and fee paying the previous semester may be seen on a fee-for-service basis. Spouses/dependents of previously enrolled students are not eligible to be seen.
D. VISITORS OF FSU STUDENTS
Visitors of currently enrolled students may be seen on a ONE TIME ONLY basis for an illness or an injury. The visitor must pay for services the day of the visit and must be accompanied by the FSU student. They ARE CHARGED for office visits* and will be charged fee for service rates.
E. TALLAHASSEE COMMUNITY COLLEGE (TCC) STUDENTS
Currently enrolled TCC students are eligible to be seen. They ARE CHARGED for office visits and will be charged the fee-for-service rates*. They are eligible to purchase a HEALTH FEE CARD** which places them in the same paying status as fee paying, currently enrolled FSU students. Spouses/dependents are not eligible to be seen.
F. WORKERS COMPENSATION INJURIES
FSU employees will NOT be seen for injuries incurred on the job. The only care provided to FSU employees will be as specified under EMERGENCY SERVICES.
G. CAMPERS
Campers are eligible for services if they are in a university sponsored camp. They will be billed for office visits and fee-for-service rate* for use of lab, nurse assisted clinics, x-ray, injections, pharmacy and other medical supplies. They must be accompanied by a camp counselor WITH A SIGNED PARENTAL CONSENT FORM, and they must also be 13 years of age or older to be seen.
H. CENTER FOR INTENSIVE ENGLISH STUDENTS
Students only (no spouse or dependents) may be seen on a fee-for-service basis*.
I. UNIVERSITY VISITORS AND VISITING PROFESSORS
Visitors to the University are not eligible for services unless they are visiting and accompanied by an FSU student or prior arrangements have been made.
J. EMERGENCY SERVICES
If anyone comes to the health center with a life-threatening problem, health center staff will stabilize the patient and dial for emergency services (i.e. ambulance). First aid will be administered as necessary.
